What Is the Rural Health Transformation Program?

The Rural Health Transformation Program was established under Section 71401 of Public Law 119-21 and provides $50 billion over five years to support rural healthcare transformation.

CMS announced first-year RHTP awards to all 50 states on December 29, 2025. First-year awards averaged approximately $200 million per state, with individual awards ranging from approximately $147 million to $281 million.

Idaho's Department of Health and Welfare lists a Healthcare Infrastructure Support opportunity as part of its RHTP implementation.

Idaho received approximately $185.97 million in first-year RHTP funding. The Healthcare Infrastructure Support opportunity provides approximately $97 million for rural healthcare providers.

Wisconsin - Rural Technology Transformation Fund

Status: Round 1 closed

Deadline: August 1, 2026

Award announcements: Expected September 2026

Wisconsin's Rural Technology Transformation Fund supports technology investments intended to improve access to care, provider productivity, and health outcomes.

Round 1 provides up to $61 million. Round 1 eligibility was limited to organizations listed in Wisconsin's Addendum Exhibit 4.

Wisconsin expects a second round in 2027, pending CMS approval. The state is also using RHTP funds for initiatives involving community health workers, care coordination, and other rural-health priorities.

Pennsylvania - FQHC Technology Funding

Status: Initial opportunity closed

Deadline: August 7, 2026

Pennsylvania created an RHTP opportunity specifically for FQHCs and FQHC Look-Alikes to modernize healthcare technology and improve interoperability.

The initial opportunity provided $1.8 million, with up to $300,000 per program and six planned awards.

The funding supported CEHRT implementation and participation in a qualifying Health Information Organization, including onboarding to the Pennsylvania Patient and Provider Network.

Pennsylvania also identifies potential future opportunities involving technology, digital infrastructure, AI, mobile and digital health, EHR/HIO initiatives, workforce, and other rural-health priorities.

Maine - EMR Modernization

Status: Initial provider opportunity closed

Deadline: August 14, 2026

Maine's RHTP includes approximately $190 million in Year 1 funding, including approximately $30 million for EMR modernization and interoperability.

The initial EMR opportunity included FQHCs, FQHC Look-Alikes, RHCs, primary care practices, hospitals, behavioral health providers, and other qualifying organizations.

Maine also established requirements involving MaineCare participation, licensing and good standing, and service to rural communities.

Although the initial EMR application period has closed, Maine plans to announce additional RHTP opportunities as implementation continues.

RHTP Eligibility: What FQHCs Should Check

Being an FQHC does not automatically make an organization eligible for every RHTP opportunity.

Before applying, check:

Provider eligibility: Does the opportunity specifically include FQHCs or FQHC Look-Alikes?
Geographic eligibility: Does the organization or site meet the state's rural-location requirements?
Site eligibility: Are requirements based on the organization, facility, or individual site?
Program participation: Are Medicaid participation, licensing, or other conditions required?
