Authorized representatives of our Customers and their organizational partners provide us with certain information when they register for the Service, such as name, mailing address, email address, and telephone number.

Customer representatives and representatives of their organizational partners must create accounts to use the Service. Customer's patients, clients, or end users must also create accounts to use the Service. We collect all information anyone provides in connection with an account. This information may include data such as name, mailing address, email address, and telephone number.

3. Information from Customers :

Our Customers and their organizational partners use the Service to store information related to their interactions with and provision of services about their patients, clients, or end users. We receive any Personal Information a Customer supplies about you such as your name, email address, mailing address, telephone number, birthdate, social information, appointment details, products and services provide to you, insurance and guarantor information, and/or payment and billing information (collectively, "Customer Data"). Customer Data is owned and controlled by our Customers.

4. Payment Information :

If Customers or their representatives provide payment information through Service, we will receive that information and share it with our payment processors to accept payment for the Service.

5. Communications and Feedback :

If you contact SocialRoots's customer support or communicate with us in any other way, we may retain the information you provide to resolve your question and/or communicate with you. Similarly, we will retain any feedback you provide to us.

2. Information We Collect When You Use the Service

1. Device Information :

1. Customers and their Partners : We collect and process your Personal Information solely on behalf of our Customers. We share your Personal Information with the Customer with whom you have a relationship to provide the Service. We may also share your information with the relevant Customer's organization partners, if instructed by the Customer.

2. Vendors and Service Providers : We may disclose information we receive with contractors, vendors, and service providers retained in connection with the provision of the Service.

3. Analytics Partners : We use analytics services such as Google Analytics to collect and process certain analytics data. 4. 2. Third Parties : Our Service may contain links to other websites, products, or services that we do not own or operate. We are not responsible for the privacy practices of these third parties. Please be aware that this Privacy Policy does not apply to your activities on these third-party services or any Personal Information you disclose to these third parties. Customer Data is retained, stored, and deleted according to our agreements with our Subscribers.

4. Do Not Track : There is no accepted standard on how to respond to Do Not Track signals, and we do not respond to such signals.

We will post any adjustments to the Privacy Policy on this page, and the revised version will be effective as of the last updated date.
